Club twists in hands during swing

My daughter has been playing for about 1 year now and recently the club twists in her hands during swing. The club is twisting in the direction of opening the club face. I don't know what is causing it and can't help her. Any thoughts?

She's hitting the ball on the toe, has a VERY soft grip, or both.
