Try practicing with chipping and pitching. I find that if I need to correct something in my swing it helps to work on it with my short game.

1) At set up make sure that with your hands on the club your leading hand, left hand for right handed players, is extended and pressing the ground also trying doing this without tension and relaxed.

2) Make sure that your hands are at the correct distance away from your body. this is usually about an open hand distance from your hands to the body. If you are chipping this maybe closer to your body.

trying with the short game and work you way up to 1/2 and 3/4 shots to see if your hits are solid. you can also try putting tape on the clubs or align the red or black paint on range ball so it will mark your iron face.

You must be pushing your arms forward as you swing or leaning forward when you start your swing. I would check to see if you are doing either of those two, and if you are, try to stop it.

I´m not really good at giving swing advice but a buddy of mine had this - we played around a bit - applying some impact tape and letting him addres the ball almost outside the toe - and he still managed to hit on the heel - off to a pro he went (since this really isnt good) and it became much better since then.

i dont see why you dont align the ball on the sweet spot at address.

There is one drill i remember for this - place a second ball just to the right of the ball you want to hit. With some people it magically cures the problem when the second ball just sits there. Sockets in it self have many causes - try this maybe it helps for you.

Probably not a good idea to try on the range. Clip it off the toe and you may hit the guy next to you, or yourself with a freak bounce.

I have the exact same problem with my driver. Worked really hard this morning getting a in-n-out swing and full extension. Managed to turn a bad slice into a less bad slice and actually managed to draw my driver once. Still, almost 90% of my shots are very close to the hosel. I am certainly not too far away. My left arm is fully extended at address and I set up on the sweet spot. I suspect I 'crunch' on my downswing and move my whole body forward slightly.
