I've been Playing Golf for: around 40 years

My current handicap index or average score is: 12

My typical ball flight is: high slice or duck hook

The shot I hate or the "miss" I'm trying to reduce/eliminate is: slice or low hook.

For the past 30 years or so i have struggled with my driver i am very inconsistent. I either hit a very bad slice that can go into two fairways over, or i can hit a very bad hook. I am just looking for a few tips and feedback to fix a problem that has plauged plauged my golf game and the enjoyment of the game. Thanks tom.

Check out this video. Looks like you lack enough rotation on the backswing and this causes your arms to lift. You have some downswing issues but cleaning up the backswing first will help.
