I had the same problem you had for the longest time and went and took a couple lessons and the two things the instructor fixed right away was how I was setting up to the ball and my grip. Once I fixed those two things I started hitting the ball straight 8 out of 10. Maybe check out some videos or articles and make sure you are gripping the club properly and setting up to the ball properly as well.
