I've been Playing Golf for: 15 years
My current handicap index or average score is: 10 handicap
My typical ball flight is: 10-15 yard fade with long clubs (5i-D), 5 yard draw with shorter clubs (6i-LW)
The shot I hate or the "miss" I'm trying to reduce/eliminate is: With my short irons and wedges I typically pull the ball about 5 yards to start but it stays on that line. With longer clubs and driver I typically push it about 5 yards and then it fades for another 15-20 yards. I'd also like to add distance, I have the muscle but I think I'm losing lots of distance with my current swing.

That's slightly better.  Let your left hip sink down into a straighter right leg on the backswing.  Like this guy:

That will get your shoulders turning down the line more and give you some leverage to drive the hips forward.
