I've been Playing Golf for 15 years, just started back up after a 5 year break. I was a 16 handicap and cannot break 100 now.
My current handicap index or average score is +30.
My typical ball flight is: average
The shot I hate or the "miss" I'm trying to reduce/eliminate is: hit off the toe and pull irons.

Half the time I hit really well straight shots and the other half is a straight pull to the left but always seems to be solid contact. I've noticed the ball marks are always on the toe of the iron. What am I doing wrong in my swing?

The reason you pull the ball is because you go from shallow to steep in transition. This is sending your path over the top. You kinda make up for it by standing up in the downswing.

Your club is outside your hands here, the path is steep and out to in.

I would work on not sending the club inward at A2. Get a swing that is more steep to shallow in transition instead of shallow to steep. This will help you create a swing path that is less out to in and help you stop yanking it left. 

As of right now you are losing your spine tilt forward because you need to create room for that steep angle. It also is a compensation to keep you from hitting fat shots.

    • Day 86: Played 18 this morning at my home course. Approach play was poor, short game was decent and kept score from getting out of hand.
    • From your description, you need to work on a consistent-ish club face at impact. Most practice swings have open faces and variable paths due to the conscious thought that goes into them, so you need to swing at a ball for us to help. 
    • thinking of experimenting with shorter shaft in driver. i know shortening the length will throw off swing weight and i will play with lead tape to see what changes i can make as i progress but my main question was about the shaft and stiffness.  i normally play a stiff so would it be beneficial to get an extra stiff 5-wood shaft to compensate for club head weight?
