Club slipping in hand

I have been plagued by a problem where i loosen my grip at impact causing the club to close and hitting either a pull or a fat shot. I know that the answer is to maintain a consistent pressure throughout the swing. I have been able to do it on short irons 8-SW but I struggle with long irons and even my Driver. Also to make matters worse I am finding that the clubs twists due to persperation. No matter what I do, new glove, dry hands I can't get the club to feel secure in my hands. Has anyone experienced these types of problems and if so what have you done to deal with it.

I know you are looking for a swing tip but you could simply try new grips - ones that are more resistant to moisture and humidity, or even larger sized ones. Maybe your grips are too small for your hands. Or, you could start playing with a Bionic glove, which I hear is awesome for exactly the issues you're describing (though not tournament legal).
