Toe Strikes / Head Falling Back (Off the Wall) During Downswing

Not the greatest video due to weather being awful lately and I know having a ball in front of me probably makes flaws more apparent but this is basically how I have been trying to do my rep lately even on the range... very slow and almost pausing at the top.

I'm consistently hitting all clubs on the toe (still on the grooves) and my typical ball flight is a push or straight. I almost never miss left or have a strike that starts left. Is this from a combination of early extension and staying on the trail foot for too long? Or is a video that's not garbage probably needed? 🙂

Nobody's going to be able to help you with that because it's a practice swing. And a shorter, abbreviated one at that.
