I've been Playing Golf for: 24 years
My current handicap index or average score is: 5
My typical ball flight is: Fade
The shot I hate or the "miss" I'm trying to reduce/eliminate is: Shot I hate is a pull.Trying to change my ball flight from a fade to draw.

What I struggle with and am trying to address: Taking the club inside on the takeaway which results in a laid off position at P4. My hands don't have anywhere to go and stay passive which results in an OTT move. 

Putting Videos: 

 

What I struggle with and am trying to address: Long Putts: Poor distance control. Doesn't seem to be a consistent miss of long or short.; Short Putts: Putts seems to come off "dead" with no energy. Misses tend to be on the low side.

Thanks for posting a video. For the down-the-line or caddy view, aim the camera along your toe lie. It’s easier to see checkpoints that way. Also film in slow motion. Most phones do this and can edit the video so just the swing part is in slow motion.

One thing I did notice is your weight seems to be on your heels instead of more on your toes.

    • Day 274 6-3 flow drill getting chest through, arms in front. Arms get a little pinned to the side, not as much in front as I want them when I add speed. 
    • Shot 48 yesterday.  For me bogey golf is good.  I was 10 over through 7 and figured with a Par 3 and 4 coming on all I needed was birdie / par to get my 45. I had a great tee shot on #8 and sunk  a 5 footer for birdie, game was coming together, now just needed par on #9. Had a great tee drive and the green was within range for a hoped GIR or nGIR.  But I pulled the shot left into tall weeds and needed to take a drop.  So much for par, but a bogey for 46 is still good for me. I hit my lob wedge to get over a small tree and saw the ball riding nicely  on line to the pin when my club hit the ball a 2nd time on my follow through causing the ball to change directions and ended up @ pin high but along the same tall weeds I just took an unplayable out of.  had no room for a backswing, Just hacked at it and it shot across the green to the rough on the far side.  Needed a chip & 1 putt got a triple bogey. you can see the hole fall apart in the screenshot below.  
    • Day 68: Quick work with some foam balls. Trying to combine not making my right leg into a pole with slightly earlier weight forward. 
    • Good range session. Key points Controlling my left knee better. shorter backswing wrist flex I still need to get better at turning while doing wrist flex stuff and the lead leg straightening. Trying to do two things at once is tricky.
    • I googled a bit and not claiming it as fact but here's my understanding.. Carl Petterssen (Mr. Five wins) = Swedish. Swedish pancakes are iconic. So someone's highly fertile brain decided that 5 wins = CP = Swedish = Swedish Pancakes. There is no other logic connecting five PGAT wins or golf in general to pancakes.
