So if anyone can point out any faults in my swing and suggest solutions to overcome the faults, it'd be highly appreciated.

It looks prety solid. If you have some slightly higher quality video recording options (I'm guessing you do and this is just the much-reduced form for online), you can submit your swing to Swing Check.
