pull/slice help

My golf game has been up and down all season, and come to think of it, for my entire life. For the most part, my shots are decent, but I tend to either pull or slice the ball when I hit it. sometimes, on longer clubs, it become exaggerated. I also notice that I come up and out of my swing when I shouldn't be. I want to know how I can correct this flaw in my swing.
I tried to upload videos, but didnt have any luck.

Well I played a round today with some people from work, and one of them comented that it looks like I jump up when I swing. Meaning that my shoulders go upwards instead of around following my hips. Is there anyway to keep myself from coming out of my swing and causing these problems?
