No, in a scramble an OB ball just isn't played. You would then have to use someone another ball which is in play.

You can, but the chances are someone in the group will have a ball in play. If all four of you hit OB, then you do it again with a stroke penalty so you will be hitting 3 off the tee.

Yuck. Hard to imagine all tee shots going OB - I would recommend the last guy hit with anything that could go straight. You could use a putter and get more progress!
