I have begun to sky some tee shots and also start fading the ball bad, not slicing but starts straight for a while then just fades off to the right; don't like it.

I think one of the problems is that when I turn my hips and shoulders, I leave my weight back on my right side and don't slide my hips forward. (I'm not sliding back, I turn and then hang back). Also I think that I'm not keeping the same plane with my shoulders coming down. I feel that staying back on my right side gets my left shoulder pulling way up and my hands get left behind me.

I am thinking this is the problem but some tips would be great. Thanks
