Do you tee up a brand new ball every round?

Definitely one per round, but on difficult courses I might need a sleeve ( or more :-( ) Fortunately, I purchase whatever is at Costco. This month it was the Callaway Hex Control. These balls are pretty good. I might buy another 8 dozen before they change it again. However, I keep them in the box until I use them. Gets awkward to keep it in my pocket.

When I happen to pull it out of the bag.  When the ball pocket is getting a bit light, I open a sleeve, mark them, and dump it in the pocket.  What comes out, and when it comes out is just random.

Absolutely not.  A ball is good enough for more than 18 holes.  I currently have about 5 18s on the ball I am using and it was a found ball to begin with.

Ditto.  Today, I'll be using my Bridgestone 330-RXS, #3, on its fourth consecutive 9-hole round.  I'll play it until I either: lose it; or it gets a significant abrasion on it.

Right now my bag is full of nothing but Penta TP3, Penta TP5 and Pro V1s that I bought from a local guy who sells "one hit wonders" extremely cheap. Anything I pull out of my bag is new and at $7.00/dozen, I have no fear or concern about losing a ball due to an errant shot.

I still never change balls though, even if I purchase a dozen at MSRP. Changing balls results in your subconscious implementing a "failure" thought. Your "just in case" action of changing balls is already decreasing your odds of success, whether you realize it or have noticed it.

Since my ball of choice isn't overly expensive (Top Flite Gamer V2 2010 model) I use a new ball to start every round.  Yesterday I opened a new sleeve and when I lost two of them by the 9th hole I used an older one still in good condition. It stayed with me through 18.  I'll keep good condition balls in the bag until I scuff one or I just feel like changing up.
