I found a thread about this topic from a couple of years ago which said to fill out this form and bring it to a Titleist seller. However, that was a couple of years ago so just wondering if the procedure is the same and if Titleist have a list of authorised dealers? Also, do any UK/US users have any input regarding the cost on top of the wedge itself? Thanks.
