The shot I hate or the "miss" I'm trying to reduce/eliminate is: pull hook or weak fade

Thanks for posting. I edited the thread title to keep it consistent with the others in this section.  It is tough to tell from this angle what is going on. You should try and get the down-the-line view to line up with your toe line. Also, a face on view would help. But it looks as though one issue would be you are not getting your weight going forward on the downswing. Look at the Key 2 weight forward videos in this thread. When you have a chance, post a face on view.

You roll the club inside, get well across the line, and then just swing down from there - steep and left.
