Can you get a down the line and a face on with a camera on a tripod. Also what is the flex of your driver shaft?

Looks to me like your grip is extremely strong, and as a result your club-face is very closed throughout the backswing.  Because of that, you're really working hard to hold it off through contact, putting your left elbow in a weird position.  Ball flight laws indicate that you did end up holding it off enough for the face to be square to the target line, but the effects of doing so had you swiping in-to-out across the back of the ball.  You also missed out on a lot of clubhead speed due to the amount of muscle tension it took to get that face square.

I think your problems start with your grip.  Get it fixed to where it's somewhere close to neutral.  This should go most of the way to fixing your club-face in the backswing.  Go the rest of the way, figuring out what to do with your wrists and hands to have the club face open in sync with the coil of your arms.  You're used to holding the club off, so you're going to have to learn to release, and supinate the left wrist through contact.  Once you're doing each of those things right, your tempo and timing (and thus your contact and power) are going to be shot to hell, because these are big changes.  The fix for that is to hit a lot of balls, focusing on your new fundamentals, until the new swing starts to feel natural and coordinated.
