All depends on the course you play and the way they have their scorecard designed. Usually its the distance from the tee to the middle of the green.

They are supposed to be from the middle of the tee to the middle of the green, but they are often from the back of the tee to the back of the green. I measure out distances on Google Earth for new courses and it is often occured that the scorecard is exaggerated in order to make the couse appear a little bit more difficult. Adding an extra five yards on each hole makes the course nearly 100 yards longer. I'm not sure why they do that. THe only reason I can think of is to get handicap services like GHIN to rate the course tougher. This means that people would post lower indexes on that course, giving them the false feeling that they play better at that course, which might make them want to come back.

Monster Tour 10.5* w/ Redboard 63

Idea Pro 18* w/ VS Proto 80s

It could be that the Google Earth measurement isn't 100% accurate. I measured out a course on Google Earth using known distances for a course that I play to see how accurate Google Earth is. Overall I was impressed, but I did find at times that the distances were off, but only by about five yards at the max.

Ask the starter; different courses do things differently.

For the most part, it is tee (defined as a plate or other marker near the tee box of the color of the tees, not necessarily where the tee boxes are that day) to center of the green, without adjusting for elevation.
