rules question

I played yesterday with my neighbor. On a Par 4 he hit his 1st tee shot (went OB) I told him to hit a provisonal (#3 off the tee correct?) he proceeded to do the same exact thing on his provisonal ball, told him to hit another tee shot (that would make #5 correct?)....he hit a nice tee ball after 2 attempts, was 155 yds out, hit a nice shot on to the green, then putted out with 1 putt.. finished with a 7 correct?

if not correct where did I go wrong in my counting? was it the provisonals.
