Number 6 doesn't make sense.. What if I only practice 5 minutes a day?

Four of your questions could be improved. Here are my suggestions (tj's questions are in boldface):

5. Normally, about how many times do you practice golf?

Consider changing underlined area to "how often."
Also, you might have to categories for this question: Practice in season, and practice out of season.

6. How long does your one practice-session normally took Change to "take."

Note what Abu said, and allow shorter time frames. Either allow for text entry of actual minutes, or possibly redo categories something like this:

  • Less than half hour
  • Half to one hour
  • One to two hours
  • More than two hours

8. Lets's say When you are not hitting the ball very well, Would you like to be able Is it going to be great if you can to contact your golf coach/instructor immediately for a lesson? and work on the problem right away? (Assume it is possible)

Consider rewording text (see above).

10. Do you want your lesson time with your golf coach/instructor to be flexible and charged according to that time?
(i.e. maybe less than one hour or more than one hour depend on what problem you need to fix)

Interesting question idea... Consider offering maybe three pricing options for lessons. Let the respondent select one. (More specific data).
