That is more the swing than your ball position.

Not really. Unless you are going from a shaft designed for a senior to something PGA Tour. Most stock shafts can be played by a high number of players.

What specific sort of swing issues do you think could be causing this?

If I have a bad day and I need it in the fairway I will put ball in middle of stance. For me I feel I hit down on the ball. Doesn't help that I have a tendency to be too quick, as well as get steep. Distance loss is 30-40 yds but usually stays in fairway for me with more fade. Moving the ball forward and high, while slowing down and shortening my backswing led to hitting much better drives, they are straighter and often they have a slight draw. Putting all thoughts aside and just relaxing has helped.

if you're going to lose 30-40 yds you might as well hit a 4 or 5 iron off the tee and be done with it. That's what I do when I'm having a bad time with the driver. if I'm hitting my driver 230 but I'm off in the woods constantly for example, hitting my 4 iron 190 - 200 and putting it in the fairway isn't going to hurt for the rest of the round. I'll do this in a competitive round.

However by this time my round is already in the toilet so I might as well try to fix that driver swing fault in the tee box by doing it the correct way. I know what the fault is. My pro told me what the fault is. I just have to make sure I focus and hit the positions.

Usually pushes and stuff like that come from the elbow getting stuck or a related swing flaw. It's something to work out on the range, not the course.

Video is more important than anything when diagnosing swing faults. Also look at other factors before you make any changes. I started hitting a high baby fade last time I was out at the range. That's usually a warning for me. I was about to make some adjustments then looked at the flags - slight headwind, but they were straight out left to right. So I could have been really hitting a draw. No changes.

@Hammer 4 mentioned how pros stay behind the ball. Here's two driver swings illustrating that:

I think it has been said already, but with a driver, you want to be making contact on your upswing.  More height that way as well!
