Chipping shanks

*Ahem* Back to the forum topic, i.e. the causes of chipping shanks.

So it seems that there is some idea that quitting on the forward pivot can cause a shank now and again. That seems reasonable, since the pivot will pull the handle of the club left causing the clubhead to work around and pulling the hosel away from the ball. Does that seem like a reasonable cause? I'm more curious if anyone knows the most common cause of chipping shanks (assuming that you don't putt it, but pivot instead).
