How do you feel physically on the back nine? Do you keep hydrated throughout your round? Do you keep stretched, especially if you ride? How's your food consumption, both on and off course?

I was out of shape when I took up golf, and would struggle the back nine. Then it became the final six. I eventually got to the point where I could play 36 without feeling fatigued, provided I had a meal between eighteens. One of the big differences for me was cardio and forcing myself to drink plenty of water.

When I ride - which is a distinct minority of my rounds - I find that I'll have a bad hole, realize that I neglected my stretching, fix that on the next tee, and play well. When I walk, it's tough to not keep my legs stretched.

There might also be a mental breakdown. It's easy to "finish" playing on the front nine by adding up your score. Then you start calculating what you have to shoot on the back nine to get a decent total score, and you put pressure on yourself to play better than you need to. Your mind set is now completely different from the one you had on the front nine, and you're toast. If that's it, learn to keep your score while not paying any attention to how it's accumulating. Not hard to do. If that's not it, then figure out what IS altering the way you think, and the solution should be clear.
