My routine:

1) Stand next to the ball and take one practice swing thinking smooth tempo.

2) Go directly behind the ball and Pick a intermidiate target a few feet in front of my ball.

3) While still behind the ball I will visualize exactly what the shot should look like.

4) Address the ball exactly the same way every time and swing!

I follow my routine like it's religion. I found a while back I would get lazy from time to time on routine shots and just make a practice swing then hit it without standing behind the ball and visualizing exactly what I wanted to do. It started to result in some sloppy shots so now I do not deviate from my pre-shot routine regardless of the situation or who I am playing with.

For chips and pitch shots I will follow my normal routine except I will usually take 3 to 5 practice swing because it's a feel shot.
