I take weekly full-swing lessons at Golftec to work on those mechanics and I play at least once a week and hit the range or my net at least once a week as well.  When I first picked this game back up I would consistently 3 or 4 putt everything and I realized it's because I spend all of my practice time working on the other parts of my game and nothing on my putting.  Now I make sure I spend at least 30 minutes 4-5 times a week putting.  Most of that time is spent in my basement hitting into one of the 29.99 auto return putting greens you can buy at Dicks, GG or Golfsmith.  While it might not simulate reading a break or anything like that it allows me to work on developing a consistent stroke and practice distance control so when I do get out there I'm not guessing as much as I used to be.  When I get the opportunity I hit up the local putting green and practice reading the breaks from both uphill and downhill putts.   I've tried a couple of different putting techniques over the last couple of months but finally settled on the claw style like Mickelson uses because for me I find that it removes the wrists and allows me to get a consistent even stroke.  Figure out what is the most effective for you and then just practice away at it until it becomes second nature.

There's no magic to it, like anything else in life you have to put the work in to get better.
