I know this is close to worthless without swing video, but just thought I'd ask.

I'm a fairly sold ball striker with all of my clubs but have a weird thing going on with my driver. I find that if I address the ball in the center of the club, I'm almost guaranteed to hit it off the heel. If I address the ball at the very toe of the club, I'm almost guaranteed to contact the center of the club face.

I'm not terribly concerned since I know that I can consistently get the same results with the adjustment at address, but I can't for the life of me figure out what kind of swing flaw might cause this (and I'm sure there are plenty to choose from). Like I said before, I'm generally a decent ball striker with all of my clubs and this adjustment just kind of befuddles me. Maybe I have some sort of Jim Furyk loop thing going on?

Posture at set up is a bit more upright than it probably should be, but this is fairly consistent across all of my clubs. Distance between body and hands is reasonable and consistent across all clubs - I don't feel like I'm jamming myself or unnecessarily reaching. My general ball flight is a light draw but misses with the driver can either be a pull (if hit really far back towards the heel) or a push slice (when I really make a bad swing at it).

If you video tape yourself from DL, look to see if your head is moving/falling toward that ball on your back swing or down swing. It's generally a balance problem. I had/have this same issue. I've spent a lot of time with my feet 6" apart and the right half way back hitting practice shots. This will help in grain better balance.

I address the ball off the toe but only with the driver. If I address the driver center clubface I feel like I'm reaching way to far. For me it's far more of a person preference and it also helps me to visualize and inside swing path.

If I miss it's always on the heel anyway, I can't remember the last time I hit a shot off the toe!

Me neither. I've even tried hitting with the ball a good 1 inch in front of the toe just to mess around and I always seem to hit center. Toe shots for me with the driver are practically impossible. And you're absolutely right about it encouraging a draw swing path. Something about the setup just makes it really easy for me to think about a nice controlled draw.

I guess it's almost like a one-plane golf swing, although I don't keep my arms on the same plane as the club.

How far up do you tee the ball?

What could be happening is that when you address the ball your driver is on the ground. When you take your swing, the driver is off the ground, which slightly moves the clubhead away from you. Does that make sense? Depends on your swing and all, and hand position at impact, but it could be something to do with it.

you're leaning forward with your shoulders. this is pretty much the only way you can accomplish this. keep your weight back and let the club swing past you.

Actually, the answer is pretty simple. You know that little mark on top of your driver that shows you where the center is? Yeah, that's not the center. They put those there so at address, the mark looks like it's in the center. Most amateurs rarely hit the center of their driver face anyway, so they never notice. If they put it at the actual center of the face, it would look well off center. When you address it, it's likely you're addressing it off the heel. So, when you felt it was on the toe, it was likely really on the center.

i can hit the center of my driver pretty good because I started taking golf serious back when we didnt use 460cc drivers. I just got a 460cc driver a few weeks ago. I use to use a very small head driver for a long time, then I got a great big bertha and I thought that thing was big back in 2000. Now look at the drivers, you cant miss with them.
