I know from the rule book that if you accidently knock the ball off the tee that no penalty is incurred as the ball is not yet in play and you can re tee but i havn't been able to find any definitive answer on these scenarios.
1. You don't use a tee when teeing off and you accidently knock the ball forward? Any shot incurred?
2. You hit your first OOB and are teeing up for your 3rd shot off the tee and then you accidently knock the ball off, is there a penalty in that case?

1, Your still "teeing off" so there´s no penalty. You should still use a tee even if it´s put the whole way into the ground. It gives you a consistent lie.

2, Your also still teeing off. No penalty

2, Your also still teeing off. No penalty

Q. A player played his original ball out of bounds from the teeing ground. Under Rule 27-1, he teed up another ball. When addressing the ball, he touched it and it fell off the tee. What is the ruling?

A. There is no penalty, because a teed ball is not in play until a stroke has been made at it (see Definition of "Ball in Play"). The ball may be re-teed (Rule 11-3). Of course, if you accidentally knock the ball off during a stroke, tough luck. ;)
