I've had this issue for a long time.  I seem to backout of the shot and my shoulders get really steep through the ball and can cause issues with over taking along with more. Here is a snapshot just after impact with a 3 hybrid

Driver: Titleist 915 D3
3 wood: 15 Callaway X Hot pro
Hybrids:  18 Callaway X Hot Pro
Irons: 4-GW Callaway Apex
project x 6.0
Wedges: 54 , 58 Callaway
Putter: 2 ball
Ball: Callaway Chrome

Frankly, looks okay to me. Hips are forward, right arm isn't fully extended, shoulder tilts are fine.

Here is a bad shot.  Its the post impact I sometimes get into and the club overtakes hard and shoulders seem steep to me.

I'd work more on your wedge and retaining straighter, longer arms than your shoulder steepness.

ha yea thanks iacas.  I think you nailed it with the lack of extension I get after impact.  Thank for the feedback
