Well... first you must purchase A LOT of land. Then you need to plot out your hole designs, get a lot of dirt to build tee boxes and green banks and such. Then you're gonna need A FRIGGIN LOT of fertilizer and grass seed. A few mowers here and there are nice too. and BAM! a golf course.

if you want to design a golf layout for fun its actually pretty simple on a program called Google Sketchup... its a free program, obviously created by Google.

on the program you can elevate and add slope to land, you can also add photo texturized aspects to simulate grass.sand

also... they have a 3d database with a wicked amount of 3d models that can be uploaded into the program... such as trees, you can design your own clubhouse, even benches and stuff like that
