I've been Playing Golf for: 16 years

My current handicap index or average score is: 9
My typical ball flight is: Fade
The shot I hate or the "miss" I'm trying to reduce/eliminate is: Slice and hook

I try to play sort of a Stack-and-Tilt-swing. Any thoughts?

Welcome to the site. First, try to position the camera to aim down the line formed by your toes and not the ball. It helps see the down-the-line view better. It is tough to tell from this angle, but your right elbow may be getting a bit stuck behind your side on the downswing. This can cause over-the-top swings at times. I've had this issue. The thread below and the two linked videos were a real help for me.

What I can see my self in the videos above is:

- My aim is to the right

- The club plane is too low in the back swing

- I dont get my left shoulder tucked enough under in the back swing, resulting in the head getting pushed back

 

I would think there is more..

First, nice view you have at this range! 

Below is a screenshot at A4, the top of the backswing. You have moved a bit back from setup, which are the yellow lines. Staying more centered in the backswing will help the path on the downswing and help make more consistent contact because it will be easier to get your weight forward.

Check out this thread. A lot of us have to work on this even though we are mid or single digit handicap players.
