Your swing goes from shallow to steep in transition. Your hips are square to the target line and your rotation stales. This causes the club to crash inward at inward. You can tell this by how low the club is when it finally shows past your body going into the finish. 

Can you post a face on view? It does look like your hips sway instead of turn in the backswing.
