The new rule allowing an alternative to stroke and distance for an out of bounds ball is easy to understand for OB drives. But, what about an approach shot that goes OB directly over the green? Do you determine the equidistant point on both edges of the fairway and set up 2 possible drop zones on each side of the fairway?

My course has two holes where "over the green" can lose a ball. The Committee wisely marked them red, anticipating the difficulty of correctly applying E-5.

You should look at both sides, but there is only acceptable Fairway Reference Point, the one closest to the Ball Reference Point, i.e. where the ball is believed to have crossed the OB line.  It may not always be clear, you have to do your best.  If you do everything that can be reasonably expected, you're fine (see Rule 1.3.B(2))
