How dumb can Chad be forgetting to register and getting DQ'd?
It's like showing up for a tournament and not checking in.

I think it was Chad Campbell but maybe not; was it him who did a similar thing in Hawaii for a tournament a year or two ago?

You are correct, it was Chad who showed up in Hawaii but had not registered for the tournament, seems like he needs to get organised.

Brandel Chamblee in 2002 was the last to not register.

The Sony Open Chad had not even committed to it. Two steps: commit (the Friday before the tournament week begins on Monday), and register (when you show up to the tournament).
