I've been Playing Golf for: I just got out of the Army a few months ago and decided to start playing golf. I have been playing and practicing consistently for the past 6 months and love it!
My current handicap index or average score is: 19
My typical ball flight is: Fairly straight
The shot I hate or the "miss" I'm trying to reduce/eliminate is: I tend to push the ball to the left sometimes and also, my ball sometimes goes sky high which causes me to loose distance.

Looks like you are bringing your wrists through quickly when you start the downswing which is causing you to flip through the ball. I would work on delaying the point at which you snap the wrist through so that at impact your hands are ahead of the ball and the shaft leaning back.

It looks as if  you had someone holding the camera for you, here is a great link, http://thesandtrap.com/b/playing_tips/filming_your_swing , about filming your swings. I would try to mount the camera on a tripod so it is steady. If you can't I'm sure it's not that big of a deal. Also I would recommend using a 6i or 7i just because that is what I've been recommend.

If you haven't checked out the 5 Simple Keys thread, http://thesandtrap.com/t/55426/introducing-five-simple-keys , it is a great start.

But from this video I would be more focused on keeping you left foot planted in the back swing and keeping a steady head through out the swing.

First, I would be mindful of your camera angles. For a Face On view you want the camera in the middle of your stance pointing at you.

You sway your hips too much. You want more of a centered pivot. Your sway is making it very hard to get your weight forward at impact.

I would check out this thread on how to make a Centered Pivot.

Yep @joeyz , your hips slide to the right on the backswing and then don't go forward on the downswing. Going to create contact and path issues.
