Swing looks a little steep and a little over the top. Your head dips a lot as well. I am not a teacher or a swing coach but these are just a couple of things that I see going on. See a teacher they will be able to straighten you out no problem. With that extension in your arms, a little more lag in the down swing and you could probably hit it a mile.

What are you hitting. It looks like a wedge. I would say your comming a little over the top, thats about it. Not a bad swing at all and i like your finish.

About all my untrained eye can see is it looks like a little bit of a chicken wing on the backswing. Might be leading to a little over the top. Great extension and a very nice swing though.
