It could be an issue of standing too close or too far from the ball. Swinging out-in can contribute. If you are able to, a video would help.

Hows your stance? Hands shouldn't be angled towards you or too far towards the ball....almost dropping straight down

I had a similar issue when I started playing a few years ago.  It wasn't my only problem, but it was noticeable.

The body will try to square up the club face and center it on the ball.  I find that in all parts of the golf swing that if I start on one side, I'll finish on the other.  E.g. start on an out-to-in swing path, finish on an in-to-out swing path.  I always tried to line the heel of the club up with the ball to try and fix toeing it.  A friend essentially hit me upside the head and said I was setting myself up for failure.  I moved the set up to the toe just to jar myself a bit and have now landed on lining a bit toe-side of center and it works great.

I'm just a hack, so take it with a grain of salt.

P.S.  One thing I like to do when I can't seem to get out of a rut is golf in ridiculous ways.  Like drop my back leg about 3 feet.  Try to swing standing on one leg.  Swing with one arm.  Swing with my eyes closed.  I find the more your brain is involved, the worse you play.

thanks for all the input guys! But @kieran123 you were right, i looked at a vid i took at the range last time, it almost seems as if im lifting my hands when im setting up, il try letting my arms drop straight at the range next time!
