I think there's some good advice above.  The two things that are key for me are first, as was said above, practice seriously.  Except for the very early warm-ups or drills where I'm just working on distance control, I want to make every practice putt.  Furthermore, I treat every putt as real as I can---once I putt it, I putt it 'til it drops, and worse than two-putt is a bad thing.  Keeping some pressure is important practice.

Second, and this was hinted at but maybe not put in the terms I normally use, but putting like any golf shot is about hitting the ball and letting it roll, not steering it.  Do your read, get your feel on distance, and then trust those.  I fall into a trap where I do all that, then at the last minute second-guess myself, usually because I'm afraid I'll blast it past the cup.  Why did I bother trying to read the putt if I was just going to do that (and usually come up 10 feet short on a 30 foot putt)?  I try to remember that if I'm going to miss, it's better to miss with authority.  I think this one falls into the same category as the advice above about not spending all day over the ball once you've read the putt.

Keep your head still and don't look up until you hear it hit the jar. Peeking leads to moving which is a sure way to miss those must makers. You can never hit enough three foot putts.
