Should your eyes be directly over the ball... perpendicular to it at address? Or should they be back from it? I guess my question is how do you measure the correct distance from the ball for each club?

For regular shots, usually the eyes are behind the ball. The head is in the center of the stance, and the ball is usually forward of center for all shots, so the eyes should be behind the ball.

As for putting, personal preference, but usually your eyes are over or slightly inside the target line, and either over the ball or slightly behind the ball.

Check out this link for the eyes.  Distance to the ball will vary on people's arm length, torso size etc but generally the butt the club is a thumb and a fist away from you.

Basically it's

- Eyes down, looking at the ball with your central vision

- Upper back soft, no sticking your butt out, no arch in the lower back

- Weight balanced between and heel and toes

- arms hang away from you slightly
