Lead shoulder too high after impact

Hey guys, so I recorded my swing today and my problem is having my right shoulder way too high after impact, along with my left shoulder being too low (left-handed). As a result, my head does not go up in the follow through, it dips down after impact and doesn't go up until very late in the follow through. How can I fix this?
