Your head moves around so much because your backswing is flat (this camera angle will exaggerate how steep it looks, and the numbers aren't necessarily accurate, but they are representative), and then to hit the ball you steepen. This steepening prevents the weight from getting forward and the hips from opening because since you've steepened, you then have to "early extend" a bit which slows the rotation.
So the fix is to clean up your backswing.
1. Flare your feet out.
2. Take your right hip up more and your left shoulder down more during the backswing.
3. Make 3/4 swings at 25% speed doing that for quite awhile.
