- Standing too close to the ball

More often it's the opposite. If you're too close to the ball you'll pull away from the ball to give yourself room. People who stand too far from the ball often try to "dive in" to get closer to the ball.

- Tugging on the club so you rotate it way open

That's not a shank, though. That's an open clubface.

- Swinging the arms too far out against the ball

Yes - but the "why" is different.

I will say this - a lot of the times in a shank the left arm is out too far on the way down. The golfer will try to uncock the club to keep it on or under the plane. Sometimes it works, sometimes it doesn't. But the left arm being out probably accounts for (educated guess here) around 75% of the shanks I've ever seen.

Would keeping the left arm connected with your chest fix that? Or is it deeper than that?

It would. Try keeping pressure in the left arm pit, put a glove or something under there while you swing. Don't try killing it under there, but make sure it stays put.

Hey guys. Any idea why a lot of my shots are off the hosel?

One reason could be that your not rotating from your spine axis (turning your shoulders), in stead your turning from your hips, thus moving (sliding) your head and shoulders backwards to initiate your backswing.
