I know it's not because of ball position because I place the ball towards the instep of my left foot(on tee).  I know if I tilt to the right(right handed) and put 60% or more of my weight on my right foot at address, then I hit up on it with no divot.

I have a 10° twist in my spine which, at address, causes my shoulders to be parallel with the ground or my right shoulder to even be higher than my left. So I think this messes with my shot making in some ways.(maybe fat shots with irons)

I know I've seen people put the ball in the middle of their stance when hitting driver. How do you not hit the ground then?

What's the best way to set up to a drive so the club doesn't hit the ground?

I change my swing every time I don't shoot par for the course.

The only time i see people take a divot with the driver are those who throw the clubhead at the ball casting it, usually with an over the top move.

Try putting it a little bit more forward (off the toe of your front foot). This might give you a little more breathing room to offset your spine issues.

- Keep your height, don't dip too much.

- Don't let the upper body (head) move forward or backward at any point.

- Slide the hips forward on the downswing, 90% weight on the front foot at impact.

- Extension through the shot.
