After studying the video many times, I think I have finally identified the cause of my towering banana slices, which is, as can be seen from the moment of impact frame below, hitting off of the toe.

I also hit all my irons consistently to the right.  I adjusted for this yesterday at the range by lining the ball up with the heel of the club, for drivers and irons, and was amazed by the results.

My question is, should I leave well enough alone and continue to line the ball up on the heel of the club? Or should I try to correct my swing. I feel myself bringing the club just noticeably inside during the swing, but I have no idea how to change that, or if I should.

Toe hits "add" draw spin (they also start the ball more to the right).

More likely: you swing too far to the left, "across" the ball, or "INward" too much (or "over the top"), which pulls the sweet spot left prematurely, resulting in toe contact.

In other words, the toe contact is the symptom, not the cause.

If I could trouble you a little more, Here is the swing I took the frame from. I don't see me swinging outside in. Am I just missing something?

Notice how low the hands are on the left.  Also the difference in the pitch of the shoulders and head in the second pic.  Good indication the path is across the ball.  How do you start to change this?  I think the shoulder turn is a little flat on the backswing causing the head to move and hips need to feel like they are pushing forward on the downswing.  I'd take a look at Keys 1 and 2 http://purestrike5sk.com/videos.php
