You need to be a lot more specific about what's going on.  Would recommend posting a video here

If you're on the practice green and hit the same chip shot close 7 out of 10 times, you're benefitting from the practice effect.

You don't get that chip or pitch shot multiple times from point X on the course - you hit the shot one time, and then you have to live with it.

As mvmac said, post a video on the other forum. You didn't say it you have poor distance control (long and short), chunking or skulling, or whatever.

Consider that when you hit the shot on the course, you're playing it in learning mode--just the way you do at the practice green. That gets you thinking about your technique, which makes you get self-conscious, which leads to you not being able to hit the shot the way you have taught yourself.  Free yourself up to do what you've learned. Give yourself permission to be good. Play the shot with the enjoyment of knowing what to do.

Yeps are common to "Choking", both will happen when a player is not able to relax during execution of a shot when needed.

To start with you will need to build confidence by practicing on course.

Pick an evening when there will be no one behind you, an hour or so before dark will work.

Hit a couple of tee shots, 2 or 3, then hit them to the green. Now when at the green, drop a couple of balls from about 20 ft from the pin. Hit a variety of chips and pitches with different wedges and bump and runs with a 9 iron.

Also, hit shots from different areas, rough, fairway, uphill, etc.

You should be able to learn to relax playing these types of shots.

Determine when an easier play will sulfide for less tense type play.

The more you practice on course, the sooner you will be comfortable to execute at any time.

Then, when the time comes when you need to play a difficult chip or pitch, confidence will prevail.
