Looks like the shoulders are turning too much.  To me it seems like that happening because you're standing too upright.  Check out this post

As Mike said, too upright, you appear to have some lower body movement, and you may be open at address. Many people are, they just don't realize it.

Agreed with the above. Something else I noticed was that your wrists are breaking at impact. I feel with more locked wrists you are able to stay on your intended line and speed much easier. Just my .02 --- I am here: http://maps.google.com/maps?ll=35.262611,-119.575171

My only problem with golf is that I am usually standing too close to the ball............ after I hit it.

Eyes more over the ball, yes. Arms more bent.

Widen the angle more and don't wear black -- and you might receive more commentary

Agreed...too tall...more flexion in the arms...keep shoulders facing the target line and don't lose the wrinkles that you see in your right wrist at address

I am usually say that if your making putts then i could care less how you putt. But personally i like to see the eyes anywere between the toes to over the ball, between that range. I like not a to wide base, that never felt good to me. I like my feet something from armpit width apart to shoulder width.

Your head and more importantly, your eyes don't even look close to being directly over the ball. Eyes over the ball is a key fundamental in putting. You should be able to address the ball and be able to drop a golf ball that is right in front of your eyes and hit the ball you are putting with.

That's one way to putt .... but some guys on tour who are known as fine putters also putt with their eyes slightly inside the ball and one ball back.

I don't think you want to be hunched over the ball but you want some bend in the knees and at the hips so your eyes are slightly inside, or over the ball.
