Bumping this with some progress, I've found that it's really hard to get my weight forward on these shots, so I set up by bumping my hips forward, almost like I'm hitting driver. I had 2 of these shots from about 100 yards out, one went right of the green, the other made it on. I really wish I had a place to practice this type of shot.

Move the ball back in your stance. This moves your weight forward relative to the ball position.

Downhill lies, move the ball more forward (as your weight is already more forward).

Your practice swing can be an indicator. Look where it starts to brush the grass. For me, it can be up to a whole ball width further back. Put the ball slightly behind that. Keep your back swing steady and don't let it cause you to sway backward.

One course I play on occasion has a Par 5 - about 500 yds. - with a 45º upslope in the main landing area.

The hole has a creek + jungle hazard to the right, and the fairway falls off to deep rough on the left.

What us lesser golfers do is hit a hybrid into the flat before the big rise. Then, take maybe a 7i uphill. The 7i will come off the hill and run down to a level area about 120 yards out. Big hitters can run it down in front of the green, but they would face a blind wedge shot out of a deep swale.

So, OP might survey the hillside upslope on his Par 5, and see if there's a little shelf or spoon area he could shoot for. This would leave a more level third shot.

I tend to move my stance so ball is closer to my back foot on downhill lie, as I like to keep my left heel planted

On uphill, I tend to move the ball forward in my stance closer to the left heel so I can lift and plant.
